Registration Information
On this page you will find links to our registration packet contents. We have provided
both Word and PDF versions of some documents. One of each document needs to be
printed to have a complete packet. All forms need to be filled out to register your child.
The information entered on the "Emergency Information and Immunization Record
Card" will have to be transferred to a "Blue Card" at Teddy Bear Care and a copy of the
child's immunization record will need to be included as well. All forms can be signed
when you come in to complete the registration of your child.
